MERAKI ACCESS REQUIREMENT FORM 2017

PART 1: APPLICATION PROCESS

This is the process by which bookable access facilities can be requested.

1. Purchase a ticket
2. Obtain a ticket reference number
3. Fill out the Access Requirement Form (only valid with a ticket reference number) and any supporting evidence.

We will endeavour to respond within 10 working days to confirm your requests. We will then send you a confirmation email.

PART 2: SUBMITTING YOUR FORM

If you cannot complete the online form, you can click here to download a PDF version and fill it out by hand.

This hard copy can either be:

Or mailed to MERAKI, 47a Bancroft, Hitchin SG5 1LA

Any supporting evidence should be photocopied or scanned and sent with your application.

Please feel free to black out any information that relates to amount of benefit paid or health conditions etc. that might be on any documents submitted.

PART 4: ACCESS PROVISION REQUEST

 A free ticket for a personal assistant (2:1 ticket) A viewing platform wristband that also grants access to stewarded toilets (comes with a lanyard to be worn by a personal assistant / companion) An Accessible Parking Pass (on production of a copy of Blue Badge) A pitch in the accessible campsite (please fill in Section 7 at the bottom of this form)

PART 5: AUTOMATIC ELIGIBILITY

A photocopy / scan of one of the following documents (dated within the past 12 months if DLA or Attendance Allowance) makes you automatically eligible for any of the above access facilities. Please check the box next to the evidence you wish to submit:

 Front page of DLA Letter (no specific rate required) Front page of Attendance Allowance Letter (no specific rate required) Evidence that you are registered severely sight impaired Recognised Assistance Dog ID card None of the above (see below)

We recognise that the evidence above is not definitive. If you do not have any of the above evidence but wish to apply for the above access facilities, use the box below to state why you require them:

You are welcome to post or email copies of any additional evidence that supports your application if you have any, or contact us to discuss your application. Each application will be judged on a case-by-case basis.

If you wish to submit any additional evidence to support your statement, please list it in the box below and attach it with your form:

PART 6: ACCESS TO THE PERFORMANCE

If you have alternative access requirements for experiencing the performance that are not covered by access to a viewing platform or standing area, please describe them below, and we shall make every effort to make a reasonable adjustment to meet your needs if possible.

PART 7: ACCESSIBLE CAMPING INFORMATION

Please note: A pitch is taken to mean for you and a PA if you require one, in one or two tents.

In addition, due to space constraints, we invite you to bring up to three additional people in up to three additional tents. These individuals will need to have purchased their own tickets in advance. If you have a family that exceeds this number, please contact us to discuss.

Remember that spaces are limited for caravans and motorhomes so it will be on a first-come-first-served basis. We cannot guarantee parking spaces in this area, unless passes have been purchased for your vehicle.

It would also be very useful if you could let us know if you will require:

 Powered-wheelchair charging point Use of a secure fridge for medication

This helps us get an idea of the demand for these services in advance.

Please inform us if you have any additional access requirements related to camping - we shall make every effort to make reasonable adjustments to meet your needs

PART 8: ACCESS DATABASE

We would like to retain data for your convenience, so that you do not need to re-submit evidence every year. If your application is successful, we can hold your data for a period of 3 years. You can then contact us to rebook access facilities without having to submit a form and evidence.

 If you do NOT want us to retain the data submitted with this form, please tick this box.

Please note that all collected data will automatically be deleted in 3 years of submission if you do not request access facilities within this time. We will not share your data with any third party organisations and you will only receive information from Meraki Festival.
